Add missing packages to publish workflow and docs

Updated CI workflow to include @fedify/cfworkers, @fedify/denokv, @fedify/hono,
and @fedify/sveltekit packages in the pre-release publication table with their
corresponding JSR and npm registry links.

Also updated the main README package table to include JSR/npm links for all
integration packages that were previously missing registry information.
